Apply for or Update your Health Care Card

How to Apply for Alberta Health Care

You are eligible for Alberta Health Care coverage if you are:

  • Legally entitled to be in Canada and plan on making Alberta your permanent home
  • Will be present in Alberta for at least 183 days in a 12-month period
  • Are not claiming residency or benefits from another province territory or country
  • Are not a tourist or visitor to Alberta (Look here to check if you are considered a temporary resident:)

Requirements:

  1. Legal Entitlement:
  2. Canadian Passport (Expired passport is acceptable)
  3. Canadian Birth Certificate
  4. Canadian Citizenship Certificate/Card
  5. Permanent Resident card
  6. Confirmation of Permanent Residence (signature of Immigration officer is required electronic or original)
  7. Federal ID Card (RCMP, First Nations/Inuit, Department of National Defense)
  8. Canada Entry Documents:
    1.             Work Permit (Alberta/Open) if the location is for another province, proof of employment in Alberta is required.
    1. Study Permit (Alberta): A study permit from another province is not acceptable.
  9. Municipal/territorial/provincial police force identification

Proof of Residency:

  • Valid Alberta ID card or Driver’s Licence (current address)
  • Bank Statement/Phone bills (The document must be mailed to you and should be dated within the last 90 days.)
  • Lease/Rental Agreement
  • Property tax Bill/Land Title
  • Mortgage Documents
  • Tenant Insurance
  • CurrentVehicleRegistration (AB)

Proof of Photo Identification:

  • Passport
  • Driver Licence
  • ID Card
  • Canadian Citizenship Card
  • Permanent Resident card
  • Federal ID Card (RCMP, First Nations/Inuit, Department of National Defense)

If you are Eligible:

  • Submit Your Application the form is available at the New Urban Registry or can be found online at https://cfr.forms.gov.ab.ca/form/AHC0102
  • The registry agent will process your application
  • If you submit in person, your processing time is 2-5 days.
  • If you choose to mail in your application, the processing can take up to six weeks. Do not send original documents by mail. You must photocopy the front and back of all documents. You can send your documents directly to Alberta Health

  • Receive Your Health Card – The Alberta Health Card will be mailed to your address once approved.

Additional Information:

  • There is no fee to apply for an Alberta Health Card.
  • You should update your information if you move or change your name.
  • If your card is lost or stolen, you can request a replacement at a New Urban Registry office

Update Alberta Health Care

When the following happens, you need to update your Alberta Health Care Insurance Plan personal information:

  • change your mailing address
  • change your name
  • change your marital or relationship status
  • have a new child or dependent
  • remove a dependent who longer qualifies to be on your account
  • report a death

Changing Address

Changing your address is simple and will only take a few minutes in a registry, we recommend when you change your address for health care that you remember to change it on all of your other Alberta documents New Urban Registry can change your address on:

  • Driver’s Licence
  • Identification Card
  • Vehicle Registration
  • Corporate Registry (Incorporation, Trade Name, Partnership)

For a Health Care change of address all you need to do is fill out the form located here: https://formsmgmt.gov.ab.ca/Public/AHC2211.xdp and visit a registry with photo ID

  • Don’t worry if you cannot print the document you can email it to us at services@newrbanregistry.com and we will print it for you!
  • You do not need to provide any proof of address just photo ID
  • You can change the address for your spouse and children by listing them as family members on the form, you are not required to bring them with you or bring their ID
  • There is no cost to change the address on Alberta Health Care.

Change your Name:

There are two reasons you may need to change your name on your Alberta Health Care:

  • Legal Name Change
  • Marriage (assuming  last name of spouse)

Legal Name Change

If you are changing your name due to a Legal Name Change you will need to fill out the form located here: https://formsmgmt.gov.ab.ca/Public/AHC2211.xdp

On this form:

  • in the top section you will fill it out completely but will use your previous name.
  • In the Section called “New Personal Information for Individual Requiring Changes” you will only fill out the information that needs to be changed on your health care card.

You will also need to bring:

  • Original Legal Name Change Certificate
  • Government Issued Photo ID
  • Completed Form (don’t worry if you cannot print it, you can email the form to services@newurbanregistry.com and we will print it for you, you can also fill out the form in the registry in person)
  • There is no cost for this service

Marriage

In Alberta, married couples are required to have a joint Alberta Health Care account.  You may already be joined on the same account if you applied for health care together and listed your relationship as “Adult Interdependent Partner” or if you visited a registry and joined your health care together because you were living in an Adult Interdependent relationship.

If this applies to you, you will follow the steps listed below if this does not you can visit Health Care Additions (hyperlink) for instructions on how to join your health care and change your name:

To change your surname to your spouse you will need to:

fill out the form located here:

On this form:

  • Complete the first section in its entirety using your previous last name (or “maiden name”)
  • In the form’s section called “New Personal Information for Individual Requiring Changes” you will use your new last name. This may be your spouse’s last name, or your current surname and spouse’s name together separated by a hyphen or a space ( in any order you like).

You will need to bring in:

  • Your marriage certificate (if not in English it will need to be translated by a document translator licensed in Alberta).
  • Government Issued Photo ID
  • Completed Form (If you are unable to print the form you may email the form to services@newurbanregistry.com and we will print it for you, you can also fill out the form in the registry in person)
  • There is no cost for this service

Change your marital or relationship status

Just got married or living in Adult interdependent Partnership (Common Law):

To join the Alberta Health Care Insurance Plan with your spouse or Adult Interdependent Partner You will need to fill out the form located here: https://cfr.forms.gov.ab.ca/form/ahc2212

  • Please make sure that if one person has children on their account and the other does not that the person with children is the applicant
  • If you were previously married or had a joint account with a previous adult interdependent partner, please make sure you delete them by visiting here (hyperlink) before adding a new spouse or adult interdependent partner
  • If you would like to change your last name due to marriage you can put your new last name in “New Last Name” box if you are the applicant or “Last Name” box if you are the spouse being added (make sure you put your previous last name in the “Previous Last Name” box so we can find you!)

You will need to bring in:

  • Completed Form
  • Marriage Certificate (if you got married or are changing your name)
  • Government Issued Photo ID for both parties
  • If your Spouse or Adult Interdependent Partner Signed the form (original signature required as this document cannot be signed digitally).  Your partner does not need to come to the Registry with you. Only the applicant needs to attend the Registry.
  • Completed Form
